here are the steps you are expected to take :
  1. receive your consent form from whoever who wants you to receive
  2. go home, and show your parents.
  3. ask them to SIGN.
  4. put it in your bag IMMEDIATELY before doing anything else.
  5. bring it to school the next day and hand in to your squad leader.
